Selecting whether to report something that has happened (e.g. an FMD, Accident or incident), or something that could happen (a hazard that could lead to a risk), ensures the reporting form only requests relevant data from the reporter.

Email notifications are sent to your assigned team members that a new report has been submitted.
Communicate with Reporter
Your team can request further information from the reporter, send files, update them on progress. All whilst protecting the identity of the reporter.
